Programs may progress through their lifecycle successfully. They may finish and be deemed a success, but what we have seen many times across government and private sector programs is that risk is nearly always mismanaged.
A failure to adequately identify risks, to assess them, mitigate them, and have plans and triggers established to respond to them, may be fatal.
